I think he used to play for Milan and he definitely played for Liverpool, Middlesbrough & Spurs. I assume he played in the relevant derbies?
The other one has got to be Paul Ince.
He played for Inter when he left Man U and I know he played for Liverpool and Middlesbrough when he came back.
He started at West Ham so he must have played other London clubs (although the true London derby games take place in North London I guess West Ham might count)
He spent his last few years at Wolves so he must have played in a few Midlands derbies as well as Manchester ones.
